Summary

Reporting directly to the Chief Executive Officer, the Medical Care Facility Administrator is responsible and accountable for all activities and departments related to the daily skilled nursing facility operations. Maintains compliance with applicable federal, state and local regulations, including Michigan’s Open Meetings Act, ensuring that all organizational policies and procedures meet regulatory standards. Qualifications

Job Summary: As an RN Case Manager, you will be responsible for the comprehensive management of new admissions, ensuring a smooth transition from admission to discharge. • Admission Processes: Complete all necessary paperwork and initial assessments for new residents, including Level of Care determinations.

• Report suspected infections to the Infection Control Coordinator.
• Check for proper placement of naso-gastric tubes and administer tube feeding as directed by physician order.

Tasks:
• Provide daily care to residents per care plan, i.e., personal hygiene, grooming, dressing, assist with dining.
• Provide restorative nursing care to include range of motion, positioning and body alignment, preventive skin care, transfer and ambulation training, bowel and bladder training.
• Encourage independence, socialization and participation in activities. 
• Notifies charge nurse of any change in resident need or condition, injury or potentially unsafe activity or event.
